I am NOT aware of any health problems that will affect my child's ability to participate in the camp. I understand that the camp does NOT provide accident insurance and that I am responsible for my child's participation in the camp. I agree to hold Vertical Vault Club, Corvallis School District, its official, agents, and employees harmless against any claim for injury or damage except in those cases where the acts of Corvallis School District, its officials, agents, and employees have been determined to be negligent by a court of competent jurisdiction. In the event I cannot be reached in an emergency, I hereby give permission to the physician selected by the coach to hospitalize and secure proper treatment for my child.

CAMP SPECIFICS
Camp cost is $45 -- All proceeds will go towards Crescent Valley pole vault equipment.
The camp is designed for elementary through college level athletes. Main emphasis will be on proper mechanics from beginner to advanced.
DATES OF THE CAMP

We're holding camp before school gets out due to the convenience of the school busing system. Students coming form Cheldelin can catch a bus which takes them to Crescent Valley High School The practices will be from 4:00 to 6:00. There will be eight sessions. You may come to all or some of the camp dates, depending on your schedule. Instruction will be individualized to meet each participant's ability level.

Check and registration forms can be mailed to my address given below. If necessary, payment may be made at the first practice. Practices will be held on the Crescent Valley High School track at the pole vault pit.
