To request a transcript, download form (using Adobe Acrobat). Fill out the request and email to Student Records.  A $3 fee will be accessed for this service.  Options include: picking up the transcript from the Registrar's office, or having them directly mailed to the school you are applying. With this form you can also order letters of recommendation from both teachers and counselors.
