Information on W-4 and W-5 Forms
IRS requires you to update
your w-4 if your filing status, exemption allowances or exempt status has
changed since your last filing. For your convenience, links are included below.
Changes Received by the 10th
of each month will be effective for that month.
Your Social Security number
will print on the November and December earnings records for your review.
Contact us immediately if there is an error in the spelling of your name,
incorrect address, or social security number.
Form W-4
- IRS
regulations require employers to remind their employees by December 1st
to file an amended Form W-4 if their filing status, exemption allowances, or
exempt status has changed since their last filing of a W-4.
- Employees
claiming exempt must complete a new W-4 by February 15th of the
new year in order to keep their exempt status ongoing.

Form W-5
- Employees
who have filed Form W-5 for advanced earned income credit payments will have
that certificate expire December 31st
- Please
contact payroll immediately if your name and social security number do not
match your social security card. The IRS will fine the District up to $50
per occurrence for incorrect W-2 information. This is critical as the Social
Security Administration (SSA) uses the W-2 information to post social
security wages to a worker’s social security earnings record. If these two
items do not match, the employee’s monies can not be posted to their
account.
It is recommended that every
2 to 5 years, workers request a statement of Social Security Earnings from SSA.
