    2012 Medical Insurance Plans


    OEBB Medical insurance plans may be updated annually during "open enrollment".  Initial plan enrollments may be made by individual employees via the OEBB online enrollment website. Plan changes may be updated, outside of the open enrollment period, whenever an employee experiences a "qualified life event" (i.e. marriage, divorce, new child/dependent, domestic partnership, etc.) by printing plan change forms. 


    2012 Supplemental Insurance: Dental, Vision, Life

    Plan enrollment, change, and claim forms may be found for the self administered CSD509J supplemental insurance benefits.  NOTE:  The Corvallis School District does not participate in OEBB options for dental, vision, Life, LTD, AD&D, EAP, etc. Therefore, all paperwork for these selections must be directed to the Insurance/Benefits Office.

    The 2011-2012 CSD509J Benefit year is effective
     Oct. 1, 2011- Sept. 30, 2012.

    The 2012-2013 Benefit year will be:
     Oct. 1, 2012- Sept. 30, 2013

    Examples of a "qualified event" include: marriage, divorce, changes in domestic partnership status,death, newborn, adoption, loss of other group coverage, and part time to full time or full time to part time. Employees must notify the Benefits Office within 31 days of effective changes!

    Don't forget to update your life insurance beneficiaries!

    Your children qualify for benefits under your plan until the last day of the month in which they turn 26.
