The 2011-2012 CSD509J Benefit year is effective
Oct. 1, 2011- Sept. 30, 2012.
The 2012-2013 Benefit year will be:
Oct. 1, 2012- Sept. 30, 2013
Examples of a "qualified event" include: marriage, divorce, changes in domestic partnership status,death, newborn, adoption, loss of other group coverage, and part time to full time or full time to part time. Employees must notify the Benefits Office within 31 days of effective changes!
Don't forget to update your life insurance beneficiaries!
Your children qualify for benefits under your plan until the last day of the month in which they turn 26.
