Immunizations Required for New Students

In order to begin attending your program, new entering students must have one of the following

  • a signed Oregon CIS form showing evidence of at least one dose of each required vaccine
  • a print out from ALERT showing evidence of at least one dose of each required vaccine
  • a fully documented valid exemption*

Examples of new entering students include

  • all children attending child care, preschool, or Head Start
  • all kindergarten students
  • all students attending a U.S. public school for the first time, from home school, online school, from outside the U.S.

Transfer students (those who attended another K-12 school in the U.S. last academic year) have a 30 day grace period to provide immunization records, but only if your school is requesting   immunization records from that school and you expect that the other school will send an immunization record. Students who are homeless also have a 30 day grace period to provide immunization records.

*Kindergarten students who had a non-medical exemption in preschool or child care are required to have a NEW non-medical exemption certificate in order to start kindergarten.
